School Board Meeting Notices
Pursuant to A.R.S. §38-431.02, Midtown Primary School hereby states that all notices of the meetings of Midtown Primary School will be posted at the Midtown Primary School offices located at 4735 N 19th Ave, Phoenix, Arizona 85015 and on our website. The location is open to the public Monday through Friday from 8:00 AM to 5:00 PM. Such notices will indicate the date, time, and place of the meeting and will include an agenda or information concerning the manner in which the public may obtain an agenda for the meeting.

Agenda available upon request 24 hours prior to any meeting by calling 602-265-5133 or visiting the campus at 4735 N 19th Ave. On all Regular and Special Meetings and Study Sessions, the agenda will list all specific matters to be discussed, considered or decided. On Executive Sessions, a general description of matters to be considered will be provided. All of the school district meetings are open to the public with the exception of Executive Sessions which are conducted in accordance with ARS 38-401.01 ETUX “If the situation warrants, an executive session may be held pursuant to ARS 38-431.03.”

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at
http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
